How to Report a Safety Issue

If you notice a safety problem, reporting it helps improve products and prevent harm.

Consumers play a critical role in product safety. If you experience a defect or safety issue, reporting it can help regulators and manufacturers identify patterns faster.

When you should report

Report if you notice:

  • A defect that could cause injury or harm
  • A product failure that seems unusual or repeated
  • Mislabeling or missing warnings
  • A medical device or medication issue

What to include in a report

A strong report includes:

  • The product name and brand
  • Model number, UPC, or lot code
  • Date and location of purchase
  • What happened and when
  • Photos (if available)

Why reports matter

Individual reports can be the signal that triggers a larger investigation. Even if your issue seems minor, aggregated reports reveal trends that might otherwise be missed.

Protect your records

Keep a copy of your report and related receipts. This can help with follow-up questions or a refund.
