How to Report a Safety Issue

Consumers play a critical role in product safety. If you experience a defect or safety issue, reporting it can help regulators and manufacturers identify patterns faster.
When you should report
Report if you notice:
- A defect that could cause injury or harm
- A product failure that seems unusual or repeated
- Mislabeling or missing warnings
- A medical device or medication issue
What to include in a report
A strong report includes:
- The product name and brand
- Model number, UPC, or lot code
- Date and location of purchase
- What happened and when
- Photos (if available)
Why reports matter
Individual reports can be the signal that triggers a larger investigation. Even if your issue seems minor, aggregated reports reveal trends that might otherwise be missed.
